Registration
Our Practice Catchment Area:
As of Wednesday 21st September 2022, and due to our increasing patient list size, our practice catchment area changed.
We found that the majority of our patient population reside centrally in the New Town area (i.e. within walking distance) and therefore we felt outlier patients would be better cared for by practices closer to them.
Our new catchment area is as follows:
- To the North - Telford Road and Ferry Road
- To the East - Newhaven Road, Pilrig Street, Leith Walk and Leith Street
- To the South - Princes Street, Shandwick Place, West Maitland Street, Haymarket Terrace, West Coates, Roseburn Terrace and Corstorphine Road
- To the West - Murrayfield Road, Ravelston Dykes Road and Strachan Road
We will continue to care for patients CURRENTLY on our list but any patient who moves BEYOND this boundary will be asked to register with a new practice.
We will also accept any relatives of existing patients residing at the same address.
How to Register:
Due to the COVID-19 outbreak, we have amended how we operate as a practice. Should you wish to register with us, we would ask that you complete the registration forms below and send them to us via the noted email address. Please attach your confirmation of address (see approved documents in the list below) and your photo ID (passport or UK driving licence). If you are from abroad, please include your BRP card. DO NOT ATTEND THE PRACTICE IN ORDER TO REGISTER. Once we have all your completed paperwork, we will respond by email to confirm your registration has been completed.
Please note that we can only offer our registration forms online at present. If you have any difficulty with this, please call us and we will arrange forms to be set aside for you to collect at a suitable time.
The practice registers patients who are residing in the practice catchment area for more than 3 months. We ask all patients to provide photo ID (Passport/British Driving Licence) and proof of address (Council Tax/Tenancy/Mortgage/Utility Bill/TV licence/Electoral roll/Home insurance). PLEASE DO NOT SEND YOUR DRIVING LICENCE AS PROOF OF ADDRESS AS IT WILL BE REJECTED. If you require any further guidance, please call the practice on 0131 260 9227 to enquire about registration.
There are two GP practices in the Health Centre who register patients on different days. If you are staying with a family member or partner, you should register with the same practice as them.
Please fill in both sets of the below forms for each person that is registering and email: loth.clinical70094@nhs.scot (Please include your photo ID and confirmation of address in the email)
