Registration

Due to the COVID-19 outbreak, we have amended how we operate as a practice. Should you wish to register with us, we would ask that you complete the registration forms below and send them to us via the noted email address. Please attach your confirmation of address (see approved documents in the list below) and your photo ID (passport or UK driving licence). If you are from abroad, please include your BRP card. DO NOT ATTEND THE PRACTICE IN ORDER TO REGISTER. Once we have all your completed paperwork, we will respond by email to confirm your registration has been completed.

Please note that we can only offer our registration forms online at present. The practice is no longer generally accessible and you will not be permitted entry in order to collect forms.

The practice is currently registering a restricted amount of patients who are residing in the practice area for more than 3 months. We ask all patients to provide photo ID (Passport/British Driving Licence) and proof of address (Council Tax/Lease/Mortgage/Utility Bill). If you require any further guidance, please call the practice on 0131 260 9227 to enquire about registration.

There are two GP practices in the Health Centre who register patients on different days. If you are staying with a family member or partner, you should register with the same practice as them.

Please fill in both of the below forms for each person that is registering and email: clinical.s70094@nhslothian.scot.nhs.uk. (If possible, please include your confirmation of address in the email or you can bring a hard copy at point of registration)

Due to high demand of registration there is a waiting list at present. The practice will reply to your email and give you a suitable week to phone us on 0131 260 9227 between 10:30am - 12 noon (Wednesday or Friday) to go over your forms and arrange a time to visit the practice to complete your registration.

We normally register patients on Monday, Wednesday or Friday mornings - though this is subject to change if required. Patients are expected to bring their photo ID and proof of address and we retain the right to refuse registration if you do not bring the agreed documents. Please bring the following with you:

If you have been out with the country for an extended period of time, you will have to confirm you are now residing in the country permanently. A contract of employment or letter stating in full time education

All new patients are invited to attend for a 'New Patient Check' with our one of our nurses should they have any particular concerns about their health. This is a medical check-up which covers:

blood pressure, height, weight, urinalysis, family and self history of heart disease

Health promotion issues: alcohol, smoking, diet, exercise, rubella status and Testicular self-exam (men only).
